A:  Bring your driver’s license or ID card from the State of California, we take CASH ONLY and any prescription medications or medical records you may have.  If you do not take prescription meds or you do not have medical records that are handy, that is ok, they are not necessary.  You will have an extensive appointment with Dr. Shore.

A:  If you do NOT have a California driver’s license or ID card, please bring a state issued driver’s license or ID card plus one of the following:  telephone bill for the home, gas or electric bill, cable bill, water/trash/sewer bill or a California DMV vehicle registration with your name and California address on it.

Q: What will I expect when I arrive?

A: You will meet Michelle at the Patient Intake window, sign in with your first name only and give her your identification along with cash or a debit card.  You will fill out the patient intake form if you are a new patient or the renewal intake form if you have seen us before.  When you are finished with your paperwork, you will bring it back to Michelle and then be seen by Dr. Shore.  If you qualify to use Medical cannabis under CA state law, we will keep the fees, if you do NOT qualify, your fee will be refunded in full.
